If you've taken part in one of our events, we'd welcome your feedback, to help us improve future events. If you haven't been sent an evaluation form by the organisers, please download, fill in and return our generic one - in Word format [Word](90 KB) (for return by e-mail) and in PDF format [PDF](30 KB) (to complete in manuscript).

If you want to send us broader feedback or enquiries about sports and leisure activities or facilities with CSSC London, we welcome this (and aim to reply as quickly as we can): please complete and submit the form below.

Please note that we can't normally deal with membership queries ourselves - which are better directed to www.cssc.co.uk where you can often do what you want to online via the website.
